It is both common sense and responsible design practice to carry out an FMEA on an item of equipment or a system whenever it is required to work in an environment where any failure mode has the potential for a catastrophic effect on the process. It is a design tool that has been around for a number of years and is used as a means of identifying single point failures or common mode failures in the design of any type of equipment, whether it is a simple widget or a complex system such as a DP system.
